長庚李威震 誤診 - 抗癌

本來覺得只是家屬氣話懶得回，
既然都叫醫生去google了，
那我只好google一下許金川醫師說了什麼。
可是google後，
也沒看到許金川醫師說肝臟骨轉移多常見，
只看到肝病防治學術基金會的文章《我有肝癌 會不會轉移？》
許金川醫師說：
當肝癌轉移到其他器官時，不一定會有症狀出現，即使出現某些症狀，也往
往不具有特異性，因此常被視為其他疾病而忽略。

所以我們不要google那麼通俗的東西，
google點醫學論文雜誌。

Journal of Cancer Research and Therapeutics 2008年的文章:
Bone metastasis in hepatocellular carcinoma: Need for reappraisal of treatment
開宗明義說骨頭不是肝癌患者常見的轉移處，
[Bone is an uncommon site of metastasis in patients with hepatocellular
carcinoma (HCC)]

World Journal of Hepatology 2014年的文章:
Palliative external-beam radiotherapy for bone metastases from hepatocellular
carcinoma.
也是在第一句說相較其他癌症，肝癌發生骨轉移的機率相對的低。
[The incidence of bone metastases (BMs) from hepatocellular carcinoma (HCC)
is relatively low compared to those of other cancers]

Annals of Hepatology 2014年的文章:
Bone metastases as the initial presentation of hepatocellular carcinoma. Two
case reports and a literature review.
說與其他癌症相比，肝癌較少發生骨頭轉移。
(Skeletal metastasis of HCC occurs less frequently compared with other
cancers)

PLOS ONE 2014的文章:
Natural history of malignant bone disease in hepatocellular carcinoma: final
results of a multicenter bone metastasis survey.
也是幾乎開頭就說了在晚期肝癌的患者身上，骨頭是不常見的轉移位置。
[Bone is an uncommon site of metastasis in patients with advanced
hepatocellular carcinoma (HCC).]

這個清單要一直列下去可能沒完沒了，
所以我們直接用有排名的文章做結尾，
Radiolody 2000年的文章:
Extrahepatic metastases of hepatocellular carcinoma.
Cancer 2011年的文章:
Hepatocellular Carcinoma With Extrahepatic Metastasis
都發現了同樣的結果，
肝癌最常見的肝外轉移位置是肺，
再來是腹部淋巴結，
第三名才是骨頭。

美國醫學教授Theodore Woodward有句名言，
聽到馬蹄聲，要先想到馬，而不是斑馬(在美國的狀況)，
醫學遇到狀況的原則就是從由較高可能性去做診斷跟下一步處置，
當較少見的事情發生而醫生的處置並沒有立刻涵蓋，
我覺得「誤診」這個帽子太大頂，
還是別亂送人戴為妙，
不然會不會出事也很難說。

順帶一提Cancer 2011年的文章的結論，
他們分析覺得肝外轉移不太會影響病患的存活率，
重要的是肝內病灶的控制，
希望能聊慰您受傷的心靈。
