親愛的版友，大家好: )

我是國立臺灣師範大學社會工作學研究所的研究生絜茹，目前在進行一份有關大腸
直腸癌病患之照顧者家庭支持、益處發現與心理調適的研究。希望透過研究使更多人瞭解
照顧者的經歷與需要，這些資訊未來也有機會作為實務工作者提供服務的依據或是政策制
定及修改的參考。

填答條件：年滿20歲、現在擔任大腸直腸癌病患家人或親友的主要照顧者

說明：

1.本問卷為匿名，填答的題目並無對錯之分，以您的真實狀況及感受填答。
2.填答時間約8到13分鐘即可完成。
3.調查期間：預計5/5(五)截止，視問卷回收狀況提前截止或延長。
